Conflict of interest statement: I was Editor-in-Chief of Diabetes Care from 1996 to 2001. Following that I became an Associate Editor with the responsibility to supervise manuscripts submitted by current editors in order to minimize any potential conflict of interest; I continue in this role today. While I was Editor-in-Chief of Diabetes Care I developed a program under the supervision of the publisher, the American Diabetes Association, to select articles from Diabetes Care to be published in Spanish and Portuguese versions of the journal. I continue to edit these editions of Diabetes Care.
I was Chair of the Data, Safety Monitoring Board of the Diabetes Control and Complications Trial and I am an expert in the design, conduct and safety monitoring of clinical trials. I have continued in this role in both government-sponsored (NIH-sponsored) and privately sponsored trials. Currently I am on the Data Monitoring Committees of the NIH-sponsored FREEDOM and ACCORD trials and I am Chair of two Data Monitoring Committees for clinical trials of insulin analogues sponsored by Eli Lilly and Company.

Conflict of interest statement: I am the first past Director for scientific affairs of the Hypertension Department of the Rio de Janeiro’s branch of the Brazilian Society of Cardiology.
I am one of the partners of a private Cardiology Diagnostic Clinic (“DIAGNOSTICOR”) where I coordinate the section of Arterial Blood Pressure Monitoring (ABPM) since this year. I am also one of the partners of a private single Dialysis Unit (clinica DERT) where I am responsible for providing chronic hemodialysis treatment for about 30 patients, mostly but not only, from the Brazilian Public Health Care System (SUS) since 1997.
I have no commercial or financial ties with pharmaceutical and device companies, public or private.

I have been supported by educational grants to attend national and international conferences by GSK, Astra Zeneca, Boeringher Ingleheim, and Chiesi. I have no investments with any pharmaceutical companies, and am neither currently, or within the last 5 years, sat on any editorial boards for journals, respiratory or otherwise. I am an ordinary member of the council of the Scottish Thoracic Society, and was the Scottish representative to the BTS Speciality Training Advisory Group in 2008 - 2009.
